A rush fee may be reasonable when a short deadline displaces planned clinical or professional work, but it should be disclosed before the assignment begins. Urgency never makes an incomplete review or unsupported opinion acceptable.

Define rush work objectively
The agreement can identify the turnaround that triggers a rush term and distinguish a genuine deadline from ordinary scheduling. Record volume and deliverable complexity matter as much as the number of days.
Confirm feasibility before accepting
The physician should identify the records available, missing evidence, legal question, expected deliverable, and time required for a careful review. Decline a deadline that cannot support responsible work.
State the premium clearly
Use a disclosed hourly premium, fixed scheduling fee, or other transparent method. Do not add an unexplained surcharge after work begins.
Preserve the same method
Rush work still requires conflicts screening, scope discipline, source review, uncertainty disclosure, and the same standards applied to a non-rush engagement.
